Squid jig fishing lure foreign body in the right external auditory canal : Case Report

ABSTRACT
Background: Foreign bodies in the external auditory canal (EAC) are commonly encountered in clinical practice. Sharp foreign bodies, however, are rarely reported and pose a unique challenge due to the risk of injury to the canal skin and tympanic membrane during extraction. Fishing lures with multiple hooks represent an unusual and complex variant of sharp EAC foreign bodies. Case Presentation: A 21-year-old male presented to the emergency department with a squid fishing lure inadvertently inserted into his right EAC. The lure featured two tiers of hooks (twelve hooks per tier) directed outward, with several hook tips embedded in the canal skin. The patient reported right ear pain without bleeding or hearing loss. Extraction was performed under general anesthesia using endoscopic
guidance. The hook tips were bent inward with forceps before the lure was carefully withdrawn. Postoperatively, skin excoriation of the outer third of the EAC was observed with no tympanic membrane perforation. A three-month follow-up, the EAC appeared clear with no complications.
Conclusion: Sharp EAC foreign bodies such as squid fishing lures require individualized extraction strategies. Bending the hook tips prior to withdrawal minimizes canal wall trauma. General anesthesia and endoscopic guidance are recommended for complex sharp foreign bodies to ensure precise manipulation and patient safety.

ABSTRAK
Pendahuluan: Benda asing pada kanalis akustikus eksterna (KAE) merupakan kasus yang sering dijumpai dalam praktik klinis. Benda asing tajam, meskipun jarang dilaporkan, menimbulkan tantangan tersendiri
akibat risiko cedera pada kulit KAE dan membran timpani saat ekstraksi. Umpan pancing dengan beberapa mata pancing merupakan varian yang tidak lazim dan kompleks dari benda asing tajam di KAE. Laporan Kasus: Seorang laki-laki usia 21 tahun datang ke UGD dengan umpan pancing berbentuk cumi yang tidak sengaja masuk ke KAE kanan. Umpan pancing memiliki dua tier mata pancing (dua belas mata pancing per tier) mengarah ke luar, dengan beberapa ujung tajam menancap di kulit KAE. Pasien mengeluh nyeri telinga kanan tanpa perdarahan maupun penurunan pendengaran. Ekstraksi dilakukan dengan pembiusan umum menggunakan panduan endoskopi. Ujung-ujung mata pancing dibengkokkan ke arah dalam dengan forsep sebelum umpan pancing ditarik keluar secara perlahan. Pascaoperasi, tampak ekskoriasi pada sepertiga luar KAE tanpa perforasi membran timpani. Pada kontrol tiga bulan, KAE lapang tanpa komplikasi. Kesimpulan: Benda asing tajam pada KAE seperti umpan pancing cumi memerlukan strategi ekstraksi yang individual. Membengkokkan ujung mata pancing sebelum penarikan dapat meminimalkan trauma dinding KAE. Pembiusan umum dan panduan endoskopi direkomendasikan pada benda asing tajam yang kompleks untuk memastikan manipulasi yang presisi dan keamanan pasien.
